What to Expect
Soon, patients will be able to self-refer and schedule appointments directly through the Meritus portal.
After referral:
- You will be contacted by a dedicated team member to begin supporting your care.
- You will receive one-on-one counseling about your medications.
- We help you understand:
- Why your medication was prescribed
- How to use it correctly
- What side effects to watch for
- What to do if you miss a dose
- How to monitor your condition and know when to follow up
Cost Support: Our goal is simple: to help you get the medications you need, without the barriers. That’s why the Meritus Rx Connect Clinic team has direct access to resources that make treatment more affordable and easier to start, including
- Manufacturer copay programs
- Foundation-based financial assistance
- Expert support with prior authorizations
- Alternative therapy review (when clinically appropriate)

Who Should Be Referred?
Patients who:
- Need prior authorization support
- Have affordability challenges or high out-of-pocket costs
- Have adherence concerns or complex regimens
- Are starting specialty medications
- Require detailed medication education. Example: New to therapies including, but not limited to, conditions like diabetes, asthma/COPD weight loss, oncology, rheumatology, neurology, and more

Our pharmacists collaborate directly with providers to:
