You and your caregiver are invited by Meritus Health Respiratory Therapists to a Peer Support Group for COPD.
Our Mission is to coach you through Self-Management of your COPD so you can enjoy your daily life.
When & Where:
- 1st Monday of the month 10 AM - 11 AM Merltus Family Medicine Walnut Street (Community Room) 24 N. Walnut Street Hagerstown, MD 21740
- 2nd Wednesday of the month 1 PM - 2 PM Meritus Pulmonary Consultants Pulmonary Rehabilitation Area (Lower Level, Side Entrance) 12821 Oak Hill Avenue Hagerstown, MD 21742
- 4th Friday of the month 10 AM - 11 AM Meritus Primary Care Meritus Medical Plaza (Community Education Room) 13620 Crayton Blvd. Hagerstown, MD 21742
What to Expect:
- Open discussion around topics that are important to you!
- Meet your peers who are living with COPD
- Demonstrations on use of medications and equipment
- Learn how to manage your symptoms effectively.
- Build relationships with your peers who are living with COPD
What to bring with you (if you have them):
- Any home device used for your treatment of COPD (Spacer device, nebulizer, PEP/flutter device, incentive spirometer)
- All of your respiratory medications and inhalers
- COPD Booklet and Stoplight tool (bring discharge folder from hospital if recently discharged)
