MyChart Patient Portal
What is MyChart?
MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:
- Request Virtual Visit (Telemedicine)
- Request medical appointments.
- View your health summary from the MyChart electronic health record.
- View test results.
- Request prescription renewals.
- Access trusted health information resources.
- Communicate electronically and securely with your medical care team.
- On-line Bill Pay.
- Ability to manage care for a loved one (proxy access).

As part of your health care team, we want to assist you in reaching your goals. Whether that means walking your daughter down the aisle, competing in a 5K, living longer or just feeling better - what matters to you, matters to us. We are here to support you! Take a minute to complete the "What Matters Most" questionnaire in your MyChart account. See below for instructions.
It can be accessed in Menu under " Questionnaires"
Once in the “Questionnaires” activity, What Matters Most is located under Optional Questionnaires.
Once you have selected the questionnaire, you will then be able to enter What Matters Most to you in the free text box up to 255 characters and submit the form.
Attention FollowMyHealth Patient Portal Users
Meritus Health will no longer add or update information to the FollowMyHealth patient portal. As of Aug. 29, you will no longer be able to send communication to your provider via Follow My Health. All of your Meritus Health records from prior to Sept. 1, 2018 will still be accessible in the FollowMyHealth portal and you will still receive health information from non-Meritus Health providers.
