HIM/Medical Records is unable to take walk-in requests for medical records at this time. If you have an urgent need, please call 301-790-8137 during office hours.

If you would like to request a copy of your Meritus Medical Group Physician Office or Meritus Urgent Care records, please contact the individual Physician office directly to request the records.

If you would like to request a copy of your Meritus Medical Center (hospital) records, please click here to download the authorization form, then mail the form to:
