Meritus Financial Assistance

Meritus Medical Center has a Financial Assistance Program available for patients who find that they are unable to pay all or part of their medical bills. This program is based on the Federal Income Guidelines of the household, assets owned by the household, and household size.

You will receive a reply within two business days of the submission of your financial assistance request. If additional information and/or documentation is required, we will contact you by phone or mail within two business days.

You may be eligible for financial assistance if you are:

  • Uninsured
  • Underinsured
  • Ineligible for a government assistance program
  • Otherwise unable to pay for medical care

Required information when submitting an application:

  • Proof of income, such as a copy of your federal income tax return or three current copies of your pay stubs.
  • If you are claiming no income, you must have a signed letter of support from the organization or person providing you with living expenses.
  • If someone in your household has an open account at the hospital and you want this account to be considered for financial assistance, provide their social security number on the form.
  • If you were denied for Medical Assistance within the last 90 days, attach a copy of the denial to your application.
