When thinking of health care, you think of receiving care from your doctors and nurses. However, it is important to know another part of your health care team—the professionals of Care Management. The Care Management department includes very experienced registered nurses, social workers, licensed practical nurses and clerical staff.

Working together, the Care Management staff provides services aimed at coordinating your hospital care and discharge plan, as well as transitioning your care back into the community . This includes communicating with your insurance, working directly with your nurses and physicians to assure your needs and concerns are being addressed, coordinating immediate follow up care needs, arranging appointments with your primary care physician or specialist and referring your care to the Outpatient Care Management team or other community services and programs for further support and disease management.

Hours: Daily 8:00am­ - 4:30pm
