are You due for your Medicare Annual Wellness Visit (AWV)?
What is a Medicare AWV?
An AWV is a one-on-one discussion with a member of your health care team, we use this visit to focus on preventative care and encourage overall health and wellness. This is covered 100% by Medicare and is different than a normal office visit.
What will be discussed during my Annual Wellness Visit?
- Medical and family history
- Health risk assessment including age appropriate screenings
- A review of your immunization schedule
- Available resources and services
Additional information
- During this visit we will not listen to your heart or lungs, although we will check your height, weight and blood pressure and inform your physician of any concerns.
- We will schedule an additional visit if you are not feeling well.
- Please bring a list of your medications with you.
