Pediatric COVID Vaccine Clinics
For the youngest age group, 6 months up until they are 5, all vaccinations will be offered at Meritus After Hours Pediatrics. Space is limited; appointments required.
To schedule an appointment, email covid.vaccine@meritushealth.com, and provide the following information:
- Name, DOB and address of child
- Name, DOB and address of parent or legal guardian
- Contact phone for parent/legal guardian
*A parent or legal guardian does not need to be the adult who accompanies the child to the appointment. However, a parent or legal guardian will need to have filled out the consent form prior, and will need to be available for a brief telephone call, during which staff will verify the child’s information and permission for vaccination.
**An important note: MyChart users may receive a notification that says to go to Robinwood, please disregard and go to Meritus After Hours Pediatrics.
