Meritus Health’s outpatient care management team, alongside numerous supportive departments and community partners, recently received a Circle of Honor 2021 Minogue Award for patient safety innovation from the Maryland Patient Safety Center.

The recognition was for readmission reduction of patients from skilled nursing homes through collaborative transitions of care. Improved communication, medication reconciliation, use of referrals and standardized workflows all led to a reduction of readmissions from eight local skilled nursing facilities from 50 in the first six months of 2018, down to 30 in the first six months of 2019.

Meritus Health was one of eight hospital/health systems in the state to receive this recognition for top safety and quality health care.
