We offer specialty care in the following areas.
Medication Education
Many patients today have multiple medications to manage. Home Health’s medication education program is designed to ease the burden of managing these medications and enable the patient to remain independent and at home. Working in close communication with the patient’s doctors and pharmacist, a skilled nurse helps the patient learn to take medications correctly to prevent possible side effects.
Medical Surgical Care
Many health conditions require either ongoing or intermittent care, either as a follow-up to a hospital visit or to treat a chronic condition. Our team cares for patients with a wide array of needs and conditions such as congestive heart failure, chronic obstructive pulmonary disease (COPD), pneumonia, diabetes complications, paralysis, and other conditions.
Wound Care
Our wound care specialists care for ostomies and a wide variety of wounds in the home setting. Working closely with your doctor, our medical staff will evaluate your condition and develop an individualized treatment plan designed to put you on the healing path. We’ll work with you every step of the way, monitoring your progress and making sure that you, your family, and/or your caregiver are familiar with your treatment plan.
Home Therapy
Our team of physical therapists, speech language pathologists, and occupational therapists take pride in helping patients restore capabilities to pre-surgery or pre-accident levels. Some of our patients reach levels of achievement that others thought impossible. Our therapists help patients with challenges related to joint replacement, motor vehicle accidents, fractures, stroke, and other conditions.
Postpartum Care
Frequently, new mother and baby are discharged from the hospital within twenty-four to forty-eight hours after delivery. Home Health’s postpartum visit program provides a skilled nursing visit the day after discharge to evaluate the newborn’s physical condition and the mother’s physical and emotional health. Visits are covered by insurance as required by state law. There is no charge to the mother or baby for the home care visit.
Home Infusion
For patients discharged from the hospital with their intravenous (IV) lines still in place, Home Health offers home infusion therapy. Under this program, the physician makes a referral to a pharmacy that prepares and delivers the medications, and a skilled nurse administers the medication in the comfort of the patient’s home.
We also have a special program called Care at Home designed for individuals who need assistance with the activities of daily living, but do not require skilled nursing care.
Care at Home
Most families want to do all they can to enable their loved ones to remain at home as long as possible. When skilled nursing care is not required but assistance with the activities of daily living is needed, we offer assistance with bathing, dressing, taking medications, preparing meals, eating, doing laundry and light housekeeping, and more. Care at Home services are generally available between 8:00 am and 8:00 pm. Many long-term care insurance policies pay for these services, or we can bill monthly. Our nurse will perform an in-home interview and assessment and provide you with a written estimate of all fees in advance.
