Employees of Meritus Health enjoy an excellent benefits program that
includes health insurance, dental insurance vision insurance, disability
insurance, life insurance, retirement savings, and paid leave time.
Time Off
Paid Time Off
Our generous time off program allows employees to begin
earning leave time with their first paycheck and use the time after
successfully completing the probationary period.
Each pay period that an eligible employee
is paid, PTO will accumulate based upon the employees budgeted FTE, length of
service (as adjusted for various reasons), classification of employment, and
organizational level. Actual accumulation rates for the 80 hours level are shown
in the table below. The calculation of each pay period's accumulated PTO amount
will be pro-rated.

Leave of Absence
We understand that there are circumstances that
may take an employee away from work for an extended period of time. To help
employees during this time, there are two types of leave of absence that
they can qualify for: Family Medical Leave Act or System leave. A leave of
absence begins with a condition lasting for more than 3 consecutive calendar
days. The maximum leave of absence granted varies by length of service
during probation, no leave of absence will be granted; after probation
through one year of service, up to 45 days may be granted; after one year of
service, up to 6 months in a rolling 12 month period may be granted. Leave
time may also be available on an intermittent basis for absences related to
a "Serious Health Condition" of the employee or their immediate family
member.
Planning for Your Future
Life Insurance
This is a great NO COST benefit for employees who work at least 20
hours per week (.5 FTE) and have been employed for at least one month.
Group Life and Accidental Death and Dismemberment each cover an hourly
employee at 1 times their annual salary and cover a salaried employee at
2 times their annual salary.
Supplemental Insurances
Employees who work at least 20 hours per week (.5 FTE) have the option to
purchase additional Whole Life, Accident, and/or Critical Illness
Insurance for themselves and family members from UNUM Life Insurance
Company and purchase additional Term Life with Aetna Life. This
benefit can round out the employee's benefit package by offering
coverage at extremely competitive prices. Rates are dependent on the
ages of each individual covered. Short-term Disability Insurance may be
purchased by the employee for him/herself. All of these options are
payroll deducted bi-weekly. These benefits are guaranteed at the time of
hire but continuing employees that decide to take the coverage later
need to meet health requirements. Semi-annual open enrollment periods.
Travel/Accident Insurance
To ensure the protection of employees whose position is required to travel,
the Health System carries this insurance at NO COST for salaried and
Home Health Care employees. The coverage provides one times annual
salary for injuries that occur while traveling on Health System
business.
Short Term Disability
Full-time employees are eligible for Group Short Term Disability
Insurance at NO COST after 6 months of employment. Premiums will be paid
by the Health System. Coverage is for 60% of your salary up to a maximum
of $1500 per month, beginning after 14 days of disability.
Long Term Disability
Long Term Disability is a NO COST benefit to employees who work at
least 20 hours per week (.5 FTE) and have been employed for at least one
month. The Health System recognizes that being off work is a hardship on the
employee as well as their immediate family so we try to minimize the
financial impact by providing 60% salary replacement up to a maximum of
$9000 per month after being off work for 90 days.
Employees Thrift (Retirement Savings 401(k) Plan)
Employees can participate in planning for retirement through our 401(k)
Retirement Savings Plan on a pre-tax or post-tax basis. Employees invest a
percentage of their pay in a select group of funds that are managed by the
T. Rowe Price and Meritus Health matches that investment. Employees have
control to change their investments as often as they want. Pre-tax
investments of 3% of pay are automatically started when the employee is
hired, unless he/she elects not to participate. Pre-tax investments are
increased automatically in 1% increments, up to 6% of pay, at the time of
the annual performance appraisal. To help advise employees on this benefit,
the plan provides financial planning consultations with Serafini Financial
Services, Inc. Appointments with a certified financial planner can be
arranged through the Human Resources Department.
Employer matching contributions.
Non-FST participants will receive an employer match in the Meritus Health
401(k) Savings Plan based on their employee deferrals. For every dollar you
contribute up to 3% of your salary, Meritus Health will contribute 50 cents,
and for every dollar over 3% up to 6% of your salary, Meritus Health will
contribute one dollar on your behalf.

HEALTH, WELLNESS, AND EMPLOYEE RELATIONS
Health Benefits
There are two economical medical plans that employees can choose to
participate in for themselves and their family members:
Network Option:
Choose any doctor within our network.
Again, choosing one is not necessary, but you might find it helpful to have
someone you can turn to who knows you and your health concerns. No
deductible and you dont need a referral when you go directly to a doctor in
our network. This plan will cover the highest percentage of all eligible
services rendered by your Primary Care Physician in his/her office, and
Primary Care doctor visits while you are a patient in the hospital.
Basic Option:
You may use any provider that you desire
but coverage will be subject to an annual deductible and covered expenses
will be reimbursed at 80% while you will pay 20%. You may also use a
provider outside the network of physician after annual deductible the
covered expenses will be reimbursed at 60% while you will pay 40%.
Full-Time
(36 hrs or more in a work week) employees may opt not to participate
in the Health Plan and receive an additional cash payment of $50.33
bi-weekly.
Prescription
Drug:
Covers up
to a 30 day supply (retail prescription). Cost determined by provider and
type of drug; generic, preferred brand, non-preferred, and specialty drugs.
Pharmacy
If an employee' purchases over the counter medications or general health
merchandise at a Home Care Pharmacy, he/she receives a 10% Discount, with
proof of Health System employment.
Dental & Vision Insurance
Option for employees and their family members to participate in the Health
System's group programs through payment of a monthly premium.
Flexible Spending Accounts
If an employee has predictable dependent care and/or medical expenses, this
benefit provides an excellent way to reduce the out of pocket expense for
these services. The chosen amount is deducted from each paycheck pre-tax and
put into an account; the employee receives a "Debit Card" to access the
funds. When employees need to make a payment, they use the Debit Card and
the full amount is automatically deducted from the account. The savings come
from taxes -- there is no Federal or State Income Tax taken out (except
Pennsylvania residents) on the amount deposited into the account. This
benefit is available to employees who work at least 40 hours bi-weekly. It
is important for you to save your itemized receipts to assure optimal
payment under this plan.
Employee Health
Employee Health offers chest x-rays and certain immunizations, vaccines and
lab tests, without charge.
Employee Wellness and Employee Activities
The Health System sponsors many activities throughout the year in which
employees are welcomed and encouraged to participate. Some of the activities
are an Employee Picnic, Night out at the Suns game, Holiday luncheon,
wellness screenings, special sales (books, purses, uniforms, and jewelry),
the Benefits Fair, and a variety of trips.
Employee Assistance Program (EAP)
Confidential assistance for a wide range of personal problems, including
marriage and family problems, alcoholism, substance abuse, financial and
emotional problems is available to employees and their immediate family
members.
OTHER BENEFITS
Promotion & Transfer
The organization posts all open positions, in the department that the
vacancy occurred first, then, if not filled, Health System-wide. A lateral
transfer within an employee's current department is available immediately; a
transfer/promotion within an employee's current Division is available after
3 months of employment. A Promotion/Transfer to a new Division/Department is
available after 6 months of employment.
Educational Assistance Program
A Tuition Reimbursement Program is available as follows:
All Full-time - Full reimbursement of tuition
costs up to a maximum program cost. This benefit is available at completion
of probationary period and completion of course with passing grade. In
exchange for this benefit, the employee is asked to repay the costs by
working one hour for each dollar paid by the Health System.
Part-time employees who fill a position authorized
to work at least 40 hours biweekly will receive pro-rated reimbursement of
tuition costs up to a maximum program cost. (Pro-ration is based upon
employee's budgeted percent of full-time.) This benefit can be applied for
at any time but actual reimbursement is only available at completion of
probationary period and completion of course with passing grade. In exchange
for this benefit, the employee is asked to repay the costs by working one
hour for each dollar paid by the Health System.
Child Care
The Learning Center has reduced rates for corporate employees who are
scheduled to work a minimum of 32 hours biweekly (.4 FTE).
Parking
The Health System provides free parking for employees in designated areas.
Reward and Recognition Program
This program allows employees to recognize each other for displaying
attributes and behaviors that connect to our core values, goals, and
principles. It offers employees the potential to be recognized and rewarded
in many ways, including thank you notes from management, peers and patients;
milestone awards for length of service and retirement.
