Before you can be seen by a doctor or therapist, you will need to schedule an intake appointment. A nurse will go over your health history, medication records, and health screening information from the following patient forms. All information is kept completely confidential. Please bring these completed forms and your health insurance card to your appointment. After the initial intake is completed, clients are matched with providers approved by their insurance and given initial appointment information.
